Troy and his team over at CCNC framed out the first pharmacy network integrating community pharmacists into medical homes. Currently, Troy’s CCNC team is piloting what looks like it’s going to be a very successful effort to electronically deliver care plans from providers to pharmacists. Just for context, there’s energy and power behind this movement to empower and enable pharmacists to support patient care. Thirty-four pharmacy networks have sprung up across the country since North Carolina’s foray.
