“We’re focused on making sure that the 80% that go home don’t have to return to the emergency room.” This week, I speak with Dr. Benji Jack about engaging patients and helping them to adhere to discharge instructions. For example, a significant number of patients fail to adhere to their treatment plan because they can’t get a ride to their follow up appointment.

It had never occurred to me before that the ER is a nexus of patients who have fallen off their carepaths from all floors of the hospital. This is certainly data we should be analyzing for macro-trends that could improve outcomes in every disease category.
